'Description': Your example does not contain a print directive for 'description'?!

'Notes': Your print directive '{#Print Class="OA" Name="notes"}' is not valid because the LDAP name of the attribute which stores the content of the Notes field is 'info'. Please try '{#Print Class="OA" Name="info"}'.
